How much can I claim?
Healthy Lifestyle benefits are included on most of our Extras cover options. The only difference is the benefits and annual limits you can claim depend on your level of cover.

*The limits detailed above are subject to a per person limit of $350 up to the policy limit of $700. The $700 policy limit is a combined limit for physiotherapy, exercise physiology, chiropractic, osteopathy, podiatry, pharmacy, complementary therapies and healthy lifestyle. Sub limits apply for healthy lifestyle with a maximum limit of $150 per person or $300 per policy. Simple options pays 60% back per claim up to sub limits.
Please note:
- A 2-month waiting period applies to our Healthy Lifestyle Benefits category.
- Benefits are payable for HIF approved programs delivered by registered providers only. Please contact us prior to commencing a program to confirm.
- The Healthy Lifestyle annual limit is combined for gym memberships, health assessments, weight management programs, quit smoking plans and skin cancer screenings refer to Extras Cover - Our Extras Insurance Options - HIF.
- A Health Management Form must be signed prior to commencing the health management program and is valid for 2 years from date of the first claim.
- A Health Management Form needs to be signed off by a qualified practitioner who can assess the need for a health management program, such as a GP or specialist (PT and Gym staff are not accepted)
To claim a benefit on your Healthy Lifestyle cover, please follow these quick steps:
- Join via the hif offer page here
- Complete program set up
- Download our Health Management Form and complete the top section;
- Take your Health Management Form to your healthcare professional (GP or Specialist) and ask them to complete the bottom section.
- Download and complete our HIF Claim Form.
Once you have all your documentation signed and ready to go, please return it to us with the invoice by either:

- Submitting your claim through our HIF Member app.
- Completing our online claims enquiry form.
- Emailing our Claims team your two completed forms and your invoice (showing payments made) to [email protected].
- Posting all your documentation to: HIF, Whadjuk Country, GPO Box X2221, Perth WA 6847.
CSIRO Total Wellbeing Diet refund offer & rebates
CSIRO Total Wellbeing Diet members who are eligible for a rebate from their health insurer are not eligible for the program refund offer which refunds membership fees at the end of the 12 weeks.
If you are eligible for a rebate through your health insurer, you will receive an invoice from us at the end of your program that you can pass on to your insurer. If you are claiming through a health fund, you do not need to meet the regular refund criteria that apply to other members. Conditions vary between health insurers, so please check with your fund for details.
